Long-Term Disability

Employees have the option of purchasing long-term disability insurance. LTD provides protection when a disability extends beyond 90 days. LTD pays 50% of annualized base pay up to a maximum of $5,000 per month.

Insurance for eligible employees is effective the first day of the month after employment with GHS.

Voluntary Vision Insurance

Voluntary vision benefits include a comprehensive eye exam, frames, lenses, contact lenses, and LASIK surgery. Coverage is available for employee and dependents. Employee has the option of purchasing vision insurance.

Voluntary Term/Whole Life Insurance

Employees may purchase term/whole life insurance as a voluntary benefit. Traditional term life insurance provides life insurance for term of employment. Traditional life insurance provides cash value accumulation based on a low fixed interest rate. Coverages may be purchased for the employee, spouse, and children.

Voluntary Accident Insurance

The Accident Plan offsets out-of-pocket costs, such as deductibles and co-payments that are not paid by your health insurance. This benefit also provides disability benefits if you or your spouse are involved in a serious accident or suffer a serious illness that results in a disability.

Voluntary Critical Illness Insurance

Critical Illness Insurance complements major medical coverage by helping employees pay for the direct and indirect costs associated with specified critical illnesses such as heart attack, stroke, major organ transplant, end-stage renal failure, and cancer.

Voluntary Hospital Confinement Indemnity Insurance

Hospital Confinement Indemnity Insurance pays for gaps not covered by health insurance, such as deductibles and co-pays. Benefits are paid directly to the employee, and there is no lifetime maximum to any of the benefits provided within the policy.

Workers’ Compensation

Workers’ compensation benefit is provided by GHS for employees with a work-related injury or illness.

All employees are eligible on the first day of employment with Granville Health System.

Flexible Spending Accounts

GHS offers a Medical Flexible Spending Account (FSA) as well as Health Spending Accounts (HSA). Employees may view all account balances, including year-to-date contributions, claims, and reimbursements, online at www.myameriflex.com. Participating employees will receive an AmeriFlex Convenience Card, similar to a MasterCard debit card, providing electronic access to FSA funds.

Medical Reimbursement – Maximum is $2,750/year.

Health Savings = Individual Maximum is $3,400/year and Family Maximum is $6,700/year.

Retirement Plans

Participation in the NC Local Government Employee Retirement System is mandatory for all full-time and part-time employees with benefits. Employee becomes a member of the North Carolina Retirement System beginning with the first day of employment at Granville Health System. Employee contributes 6% of gross pay pre-taxed.

NC Supplemental Retirement Plans
GHS employees may make elective contributions to a 457(b) deferred savings plan and 401(k), subject to governmental limitations.

Paid Time Off (PTO)

Full-time and part-time with benefits employees accrue PTO within their first day of employment. For non-exempt employees, PTO is available for use after 90 days of employment. There is no waiting period for exempt employees. Unused balances at the end of the year will carry over to a maximum of 320 hours (40 days). Part-time with benefits employees accrue one-half the benefit of a full-time employee. GHS will pay out PTO when employment terminates, provided the employee gives and works their proper notice.

Extended Illness Leave

Full-time and part-time with benefits employees accrue extended illness hours within their first day of employment. Part-time with benefits employees accrue one-half the benefit of a full-time employee. Employees can begin using extended illness hours after 90 days as a full-time employee and six months as a part-time employee. Unused balances carry over with no maximum accrual. There is no payout of sick leave when employment terminates.

Bereavement Leave

Full time employees are eligible to receive up to three days (or 24 hours) of bereavement pay in the event of a death in the immediate family. Immediate family is defined as husband, wife, son, son-in-law, daughter, daughter-in-law, mother, mother-in-law, father, father-in-law, brother, brother-in-law, sister, sister-in-law, grandparent, or grandchild, or any step or foster relations in any of these categories.

Jury Duty and Subpoenas

An employee who must appear in court for either jury duty or as a witness is provided paid time for participating in this community service.

Employee Assistance Program

Employees and their immediate family members are eligible to take advantage of professional, confidential counseling services up to three sessions per year at no cost.
